Fields on the Member Details Form


Refer to the following tables for descriptions of the fields on the Member Details form. An asterisk (*) indicates required information.

Some of the lists contain many options, which are listed in alphabetic order. To find the option you want, type the first few letters into the field. As you type, options matching the letters appear in the list. When the option you want is displayed, select it or press Enter.

To make changes you must put the form in edit mode. To do this, click .

Personal Information

Field

Description

Gender *

Member's gender. Select one: Male, Female, Unknown.

Title

Member's title (for example, Dr., Mrs., Deacon).

First Name *

Member's first name.

Nick Name *

Member's nickname.

Middle Name

Member's middle name.

Last Name *

Member's last name.

Suffix

If applicable, select the member's name suffix (for example, Jr., III, M.D.)

Maiden Name

Member's maiden name.

Role *

Select the member's role:

  • Designate at least one adult in the family as Head, Husband, or Wife. In a family with two adult members, you can assign both adults the role of Head.

  • For members having a role other than Head, Husband, or Wife, determine the role to assign by thinking about the member's relationship to the primary adults in the household. For example, a parent who lives with a son and daughter-in-law would be assigned the role of Mother or Father.

Birth Date

Type the birth date or click and then select the birth date from the displayed calendar. For instructions on using the calendar, see How to Use the Calendar to Set a Date.

Birth Place

Member's place of birth. Click to open the Postal Code Search window and locate the birth place city, state, and country.

Age

Non-editable field. The Age field is updated by the system based on the information supplied in the Birth Date field.

Religion

Member's religion.

Language

Member's native language.

Ethnicity

Member's ethnic group or affiliation.

Marital Status

Member's current marital status.

Envelope #

Non-editable field. The system displays the member's envelope number assigned by the church.

General Tab

Field

Description

Education & Career

Career Type

Member's profession. For children, select Student or leave blank.

Details

Use this field to provide information related to the member's career (for example, member's employer).

School

Name of the school the member attends or attended.

Education

Highest education level achieved by the member.

Grad Year

Year the member graduated from high school.

The system automatically calculates the value in the Grad Year field as 18 years after the value in the Birth Date field. If necessary, you can change the system-supplied value.

Additional Information

Soc. Sec #

Member's social security number.

If editing the social security number, you must re-type the entire number into the field. The system does not allow you to edit individual digits in the number.

Religious Education Tab

This tab retrieves information from the member's religious education records.

Term of enrollment.

Start

Start date of the term.

End

End date of the term.

Class

Name of religious education class.

Grade

Student's grade level.

Present

Class attendance statistic: % present.

Absent

Class attendance statistic: % absent.

Attendance

Click the Full Record link to view and print the student's attendance records.

Contact Tab

Field

Description

Contact Information

Email Address

Member's e-mail address.

Home Phone

Member's home phone number.

Cell Phone

Member's cell phone number.

Work Phone

Member's work phone number.

Pager

Member's pager number.

Fax

Member's fax number.

Sacraments Tab

This tab lists the seven sacraments and shows the following sacrament information:

       Completed: the checkbox indicates whether the sacrament was completed (checkbox selected) or not (checkbox not selected).

       Date: shows the date the sacrament was completed.

       Prep Year: shows year in which preparations leading up to the sacrament took place.

       Parish: shows name of the organization where the sacrament took place.

       Celebrant: shows the name of the celebrant.

Misc. Tab

Field

Description

Member Workgroups

This section lists any workgroups the member belongs to. This information is retrieved from the member's workgroup affiliations in Family Directory.

Member Strengths

If your organization is keeping a profile of member strengths, this section shows up to five strengths.
